The skin surface is left entirely undisturbed; the energy passes through it and converges below, the way light converges through a magnifying glass.
At 4.5 mm the treatment reaches the superficial musculoaponeurotic system, the SMAS: the fibrous layer of tissue that a surgeon lifts and repositions during a facelift. Heating it in controlled points triggers a wound-healing response, and over the following months the body lays down new collagen and elastin along those points. The tissue contracts and firms from within.
Key Point: Ultherapy does not remove skin or reposition it. It prompts the deep layer to rebuild and tighten on its own. That is why the result is a lift measured in millimetres rather than centimetres — real, visible, and modest by design.
The part that distinguishes it
Ultherapy is the only device of its kind that lets the practitioner see the tissue in real time. The handpiece produces a live ultrasound image of the layers beneath it, so your injector is not estimating depth — they are watching it. That matters in two directions: it means energy is placed exactly where it will do something useful, and it means bone, nerve pathways and areas of unusually thin tissue can be actively avoided.
It is a meaningful safety and accuracy advantage, and it is also the reason the operator matters so much here. The same machine in two different hands does not produce the same result.
Who Ultherapy Suits
The best candidates have mild to moderate laxity — skin that has started to descend but has not yet developed significant excess. Practically, that tends to mean patients from their late thirties into their early sixties, though skin quality matters far more than the number.
The areas it is most used for are the brow, where a subtle lift opens the eyes; the lower face and jawline; beneath the chin; the neck; and the décolletage, where it softens the crêpey horizontal lines that sun exposure produces.
When it is not the right treatment
If there is substantial loose skin, a heavy jowl, or significant fullness under the chin, Ultherapy will under-deliver and a surgical consultation is the more honest recommendation. We would rather tell you that than take your money for a disappointing result. It is also not the treatment for volume loss — that is a Sculptra or filler conversation — nor for dynamic expression lines, which remain Botox territory. Pregnancy, active infection or open lesions in the treatment area, and severe cystic acne in the area are all reasons to postpone.
What the Appointment Is Like
Before
There is very little preparation. We ask patients to avoid blood-thinning supplements and alcohol for a couple of days to reduce bruising, and to come with clean skin. Because comfort is the honest sticking point with this treatment, we discuss analgesia at consultation and plan it properly rather than improvising on the day.
During
A full face and neck treatment takes around sixty to ninety minutes; a single area such as the under-chin can be done in thirty to forty-five. Ultrasound gel is applied, the treatment areas are mapped, and the handpiece delivers lines of energy across each zone while the practitioner watches the imaging.
We will not pretend the sensation is nothing. Patients describe brief deep prickling and heat as each line is delivered — a sensation that comes in waves and stops the moment the pulse does. It is felt most over bony areas and along the jaw. It is manageable, and it is genuinely brief, but a clinic that describes Ultherapy as painless is not describing Ultherapy.
After
You can go straight back to your day. Most patients leave slightly pink and mildly swollen, and that settles within hours. Some tenderness to the touch is common for a few days to a couple of weeks, and occasional small bruises appear where energy was delivered over bone. There is no dressing, no peeling and no social downtime.
Timeline: A subtle initial tightening may be visible immediately, but it is not the result. Real change begins around eight to twelve weeks as new collagen forms, and continues to build to a peak at roughly six months. Most patients maintain with one session a year.
Ultherapy Alongside Other Treatments
Ultherapy works on the scaffolding. It pairs naturally with treatments that work on the other layers, and most of our patients combine rather than choose.
Botox continues on its usual schedule — there is no conflict, and a Botox brow lift and Ultherapy can be genuinely complementary around the eyes. Microneedling refines surface texture and tone above the structure Ultherapy has rebuilt, and is usually scheduled a few weeks apart rather than the same day. Where volume rather than laxity is the issue, Sculptra or filler answers a question Ultherapy cannot.
The sequence matters. We generally treat with Ultherapy first, let the collagen response declare itself over a few months, and then assess what — if anything — still needs addressing. Patients are frequently surprised by how much less filler they want afterwards.
Safety and Realistic Expectations
Ultherapy has a long clinical track record and a well-characterised safety profile. Side effects are usually limited to redness, swelling, tenderness and bruising. Less commonly, patients experience areas of numbness or small firm patches under the skin, which resolve over weeks. Temporary weakness in a facial muscle is rare and, when it occurs, resolves — and it is precisely the risk that real-time imaging exists to minimise.
The more common disappointment is not a complication but a mismatch of expectation. Ultherapy produces a natural, gradual firming that people around you register as looking well rather than as having had something done. Patients who arrive wanting a visible, immediate change are the ones who leave unhappy, and the failure there is the consultation's, not the technology's.

How long do the results last?
Typically around a year, with most patients choosing one maintenance session annually. The treatment does not stop ageing; it resets the starting point and then ageing resumes from there.
Does Ultherapy hurt?
There is real discomfort during delivery — deep heat and prickling, in short waves. It ends with the treatment and there is no lingering pain beyond tenderness. With planned analgesia, the overwhelming majority of patients complete a full session comfortably.
How many sessions do I need?
One, in most cases. A second session is occasionally recommended at six months for patients with more advanced laxity, assessed once the first result has fully matured.
Can I have Botox or filler at the same appointment?
Not in the same session over the same area. We typically stagger them by a couple of weeks so that each result can be assessed independently. Your practitioner will plan the order at consultation.
Will anyone be able to tell?
They will notice that you look rested and that your jawline looks defined. They will not be able to identify why, which is the entire point of the treatment.
